Return Goods Authorization Form

Rayplex Limited will not accept any unauthorized returns. A return number will be furnished by fax or email and must appear on the outside of the package. You must have the invoice number, date of purchase, part number, and the price you paid for the merchandise in order to receive an authorization number.

A 15% restocking fee may be applied to incorrectly ordered, incomplete or damaged product.
An RGA # MUST be obtained prior to shipping.
Show RGA # on all packing slips and labels.
No fault found product will be returned.
